Health & Wellness Benefits

**Medical Insurance:

  • Eligible employees can choose between two plans: PPO or High Deductible with a Health Savings Account. The PPO plan has an annual deductible of $1,500 for single coverage and $3,000 for family. The HDHP plan has an annual deductible of $3,500 for single coverage and $7,000 for family. The effective date of benefit is the first of the month following the date of full-time employment.

Health Savings Account:

  • Employees enrolled in a High Deductible Health Plan can contribute pre-tax dollars to a Health Savings Account. Aspire funds Health Savings Accounts for eligible employees, $1,500 for employee only coverage and $2,775 for family coverage. Health Savings Account contributions are pro-rated based upon the effective date of coverage. The effective date of benefit is the first of the month following the date of full-time employment.

Dental Insurance:

  • Dental coverage is available to eligible participants. Aspire offers two plan options that have no deductible and provide 100% coverage of in-network diagnostic and preventive services. The Basic Dental Plan offers a $1,000 Calendar Year Benefit for each plan participant.  The Buy-Up Dental Plan offers each plan participant a $2,000 Calendar Year Benefit. The effective date of benefit is the first of the month following the date of full-time employment.

Vision Coverage:

  • Vision coverage is available to eligible participants who enroll in one of the medical plans.. Services include $10 copay on routine eye exams every 12 months, $25 copay on select lenses and $130 allowance on contact lenses every 12 months or frames every 24 months. The effective date of benefit is the first of the month following the date of full-time employment.

Prescription Coverage:

  • Prescription drugs are included in the PPO and HDHP medical plans which are available at the retail pharmacy or can be utilized through the discounted TrueScript mail order program. Aspire Indiana also provides a low cost Rx program that allows employees to receive select prescriptions at a 90 day supply for $4. These programs are available to eligible members enrolled in the health plan. The effective date of benefit is the first of the month following the date of full-time employment.

Flexible Spending Accounts:

  • The Health Care, Limited Expense Health Care, and Dependent Care Flexible Spending Accounts can help defray out-of-pocket health, dental and vision care expenses as well as Dependent day care by allowing you to set aside pretax dollars to pay for them. By using pretax dollars payroll deductions for eligible health care and dependent day care expenses, you increase your disposable income by reducing the amount you pay in taxes.  The effective date of benefit is the first of the month following the date of full-time employment.

Low-cost healthcare through Aspire Indiana Health:

  • Aspire offers its employees on-site Health and Wellness Centers where primary care teams are focused on maintaining and improving your health.  On-site locations are available at Elwood, Anderson, Carmel, Lebanon, Noblesville, and Indianapolis. Some of the many benefits Aspire Indiana Health may offer at very low cost to you: medications, lab tests, vaccines, smoking cessation, cancer screening tests, HIV rapid testing, Hep C rapid testing, referrals to specialists, and much more.

Employee Assistance Program:

  • Confidential counseling and referrals to help you and your family manage life’s challenges. Aspire provides, through a facility other than its own, confidential counseling and referrals to help you and your family manage life’s challenges. This program is at no cost to employees and eligible dependents, and is available on the first day of employment for all staff.

Life Insurance:

  • Providing family security in the event of your death is another aspect of benefits. Eligible employees currently receive Basic Life Insurance & Accidental Death and Dismemberment coverage for a death benefit equal to one times annual salary up to $200,000. Additional Life and AD&D insurance is available to purchase for employees and their dependents. The effective date of benefit is the first of the month following the date of full-time employment.

Disability:

  • Aspire provides eligible employees Short-Term Disability benefits for an employee’s own medical condition. This benefit is utilized for disabilities from 8 days to 180 days. During the 1 week waiting period, employees will use available PTO, or no pay if PTO is exhausted. Benefits paid are determined by years of service. The effective date of benefit is 1 year from the first day of full-time employment. Aspire provides eligible employees core long-term disability insurance that pays a benefit of 60% of the employee’s monthly base salary up to $8,000 per month.  This plan requires a 90 day waiting period before disability benefits are payable. Employees also have the option to buy-up supplemental long-term disability insurance for an additional premium. The effective date of benefit is 1 year from the first day of full-time employment.

Workers' Compensation:

  • Payment of approved medical expenses and lost wages from an illness or injury incurred in the course of your employment in accordance with Indiana law; provided at no cost to all employees.

Tobacco-free Workplace:

  • Aspire is committed to providing a safe and healthy workplace and promoting health and well-being of our employees and clients/consumers. Accordingly, all Aspire locations/campuses are smoke-vape- and tobacco-free zones.

Financial Planning

Retirement Program:

  • On the first day of employment, each staff member who works a minimum of 20 hours is eligible to participate in Aspire Indiana’s 403(b) Retirement Plan. Automatic enrollment in this plan occurs for all qualifying employees following 18 days of employment at a rate of 3%.  Those who do not wish to participate have the option to opt-out at any time. Voluntary employee contributions are made through payroll deductions with pre- or post tax dollars, as elected. After one (1) year of service, Aspire provides a 100% match of employee contributions up to a maximum of six percent (6%) of employees eligible wages. Employees must work a minimum of 1,000 hours during the plan year to be eligible for the employer matching contributions.

Time Off

Paid Time Off:

  • Paid Time Off (PTO) is a benefit provided by Aspire Indiana for the purpose of providing eligible  employees with the opportunity to take time away from work without loss of compensation. The PTO benefit is an inclusive time-off program intended for use in connection with vacations, short-term injuries, illnesses, personal business, family care, school activities, emergency closings, appointments, and any other needs that may require time off from work. Payment of PTO should not be confused with payment of other paid leave programs (holiday pay, short-term disability, jury leave, or bereavement benefits.) - This benefit begins at the start of employment.

Holiday:

  • Aspire recognizes the following Federal holidays: Full-time employees are eligible for Holiday Pay for all Aspire recognized holidays: New Year’s Day, MLK Day,  Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving (Friday), Christmas Eve, Christmas Day.

Jury Leave:

  • Employees are provided jury leave pay with official court jury summons. 

Bereavement Benefits:

  • Aspire recognizes the need for time away from work in instances of personal loss. Should a death occur in the immediate family of a staff member, the staff member is eligible for time off according to Aspire’s bereavement leave schedule.

Leave of Absence:

  • Aspire understands that employees may experience the need for extended time off from work for certain family and medical reasons.  For those employees who meet eligibility guidelines, the company supports leave requests under the Family and Medical Leave Act of 1993 (FMLA), the Indiana Military Family Leave Act (MFLA), and the Uniformed Services Employment and Reemployment Rights Act (USERRA). For employees who do not meet criteria for participation in these programs, the company may consider a personal leave of absence (LOA).

Education Expense Retirement Program:

  • Qualified employees can receive a quarterly contribution to their 403(b) Retirement Plan. The contribution is $250 for those making payments on a technical certificate, associate's, or bachelor's degree, and $500 for those making payments on a graduate degree, post-graduate certification, or higher degree.

  • Employee referrals are an effective way for recruiting and retaining top-notch employees.
